HOW TO USE THIS FORM

  • Fill in this form before the hearing as a proposed order
  • Bring it to court — the judge or Clerk fills in findings and signs
  • Ask for certified copies immediately after the order is signed (Clerk charges a fee per copy)
  • Get at least 3–4 certified copies for Vital Records, school, SS Administration, and passport

AFTER THE MINOR NAME CHANGE ORDER

  • 1. Birth Certificate — NC Vital Records (DHHS), bring certified order. This is the most important update for a minor.
  • 2. School Records — bring certified copy to the school registrar
  • 3. Social Security Card — SSA.gov, free
  • 4. Passport — DS-11 if first passport; DS-82 for renewal
  • Note: Divorce decree that restores a mother's maiden name is a different process — check the divorce decree language, not this form
