- US Passport — Form DS-5504 (if within 1 year of issue) or DS-82

IMPORTANT SEQUENCE
- 1. Obtain signed SP-87 Name Change Order from Superior Court
- 2. Present this SP-91 form to Clerk's office with $10 x copies requested
- 3. Clerk affixes seal and signature — certificate is now official
- 4. Update Social Security Administration FIRST (Form SS-5)
- 5. Use SSA confirmation letter + certified certificate for all other agencies
