  • Social Security Administration (Form SS-5) — update SSA FIRST before all other agencies
  • NC DMV — driver's license and vehicle title
  • US Passport — Form DS-5504 (if within 1 year of issue) or DS-82
  • Employer records and payroll
  • Bank accounts and credit cards
  • Voter registration (NC State Board of Elections)
  • Professional licenses (NC licensing boards)
  • Each agency needs a certified copy — request multiple at $10 each
  • Name change is effective as of the court order date, not the certificate date

IMPORTANT SEQUENCE

  • 1. Obtain signed SP-87 Name Change Order from Superior Court
  • 2. Present this SP-91 form to Clerk's office with $10 x copies requested
  • 3. Clerk affixes seal and signature — certificate is now official
  • 4. Update Social Security Administration FIRST (Form SS-5)
  • 5. Use SSA confirmation letter + certified certificate for all other agencies
