If bond is not waived, the executor must post a surety bond with the Clerk. See G.S. 28A-8-1.

I, [Executor Name], being duly sworn, certify that the above-named decedent died on the date stated; that the instrument submitted herewith is, to the best of my knowledge and belief, the last Will and Testament of the decedent; and that I accept the duties of Executor of this estate pursuant to G.S. 28A-6-1.
FILING INSTRUCTIONS
- File the original will + certified death certificate + this application with the Clerk of Superior Court in the county where the decedent was domiciled at death
- Filing fee: approximately $120 in Wake County (G.S. 7A-307) plus $6/page for will
- The Clerk issues Letters Testamentary — this form is the application/request, not the letters themselves
- Letters are typically issued same day in most NC counties
- Bring valid photo ID for the executor
